the Music and Entertainment for your event the following forms are here to help you. Please refer to the welcome
email we sent you when you booked our service, it has your login information for this page.
To begin, click on the event that you are having below. A dialog box will appear requesting login information.
Type in your login information and click submit, your form will appear. Complete this form with as much of your
information as possible, then click submit.
Remember your planning form should be completed and submitted to us "As Soon As Possible" or "No Later Than 3 Weeks Before Your Event". |
